Most importantly, Diane took the lead in managing my mother’s medical appointments. As a registered nurse, she effectively communicated with doctors, understood my mother’s health requirements, and scheduled all her appointments, significantly easing our burden. After each visit, Diane kept us informed, ensuring we were always updated on my mother’s care.

Diane Dennis provided Senior Case Management for my mother when she was diagnosed with cancer. Diane became her Medical Power of Attorney, acted as her health advocate, consulted with her doctors, accompanied her to physician visits, explained complex medical issues, managed her medications, set up hospice care, caregivers, and made it possible for mom to have her last wish, to stay in her home.
Diane oversaw and managed her private caregivers, managed her mediations, delegated her meds to caregivers, and worked with our family to help us understand mom’s prognosis, diagnosis, and stages during her end of life care.
We would not have been able to keep mom in her home, if it weren’t for Diane working as her Senior Care manager, Medical Power of Attorney, and health advocate, making sure her medical, social, emotional and mental health needs were met.
This was an invaluable service, and our family is grateful for having Diane support mom and all of us throughout the difficult transitions in mom’s care and end of life needs.
